About Our Practice • Medical Practice located in Midtown Sacramento (2nd house on the Northwest corner of 21st & P Street).  Parking available behind the building or 2 hour street parking easily accessible. • Office Visits by Appointment Only • Established patients should arrive at least 15 minutes prior to their appointment time.  New patients should arrive at least 30 minutes prior to their appointment to complete registration information. • All fees, copays or outstanding balances must be paid in full prior to rendering of medical services. • A $25.00 fee will automatically be charged for appointments not cancelled 24 hours prior to the scheduled appointment time. • Call for any explanations about our fee structure or policies.
